Please remember to bring the following items on your first visit.

• New patient registration form + Parent Acknowledgement (if patient is 17 or younger): These forms can be printed and filled out prior to your first appointment to ease the registration process. Please find our new patient forms below. If you are not able to print + fill these out, you can fill it out in the office prior to the appointment.
• Current health insurance cards: Please provide both the primary and secondary cards, if applicable.
• Valid form of identification: An example of this would be your driver's license.
• Relevant Health Records and Reports: This can include CT scans, MRI scans, X-rays, lab reports, etc. Please bring any CD's if possible.
• Co-pays for your visit
• Questions, comments, or concerns you may have.
